NDIS Support Coordination

Getting the Most Out of Your NDIS Plan

When you have an NDIS plan you have choice and control over which servicers providers you would like to connect with. If you’re not able to coordinate your own services, Support Coordination may be included in your plan to assist you to find supports and services that are the right fit for you. 

Challenge Community Services is a fully registered NDIS provider for Support Coordination services. Our experienced team can work with you to coordinate NDIS supports, organise informal services, resolve points of crisis and help you develop important connections within your community. 

How Can a Support Coordinator Help You?

Support Coordinators are here to help you get what you need to meet your goals and get the most out of your allocated NDIS funding. 

An NDIS Support Coordinator will support you to understand the funded supports in your plan and link you to community, mainstream and other government services. A Support Coordinator will focus on supporting you to build skills and direct your life as well as connect you to providers. 

Your Support Coordinator will assist you to negotiate with providers about what they will offer you and how much it will cost out of your plan. Support Coordinators make sure service agreements and service bookings are completed. They will help build your ability to exercise choice and control, to coordinate supports and access your local community.

They can also help you prepare for your NDIS plan review meetings. 

Make the Most of Your NDIS Support

Challenge Community Services support coordinators offer two different levels of Support Coordination that can be included in your plan: 

  • Support Connection – This support is to build your ability to connect with informal, community and funded supports enabling you to get the most out of your plan and pursue your goals. 
  • Support Coordination – Coordination of supports: This support will assist you to build the skills you need to understand, implement and use your plan. A Support Coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community. 

Depending on your individual goals, plan objectives and aspirations you may receive funding for these supports in your plan.

How Challenge Support Coordinators Work With You

Our team of experienced Support Coordinators are passionate about helping people get the most out of their NDIS plans. They are dedicated to getting to know you and what you need to help you live the life you’ve always wanted to. 

To make sure you’re always getting the support, your Support Coordinator will: 

  • Work with you to understand your NDIS plan, help you identify your goals and desires, and inform you about the services available in your area. 
  • Connect you with community, mainstream and other disability services that are right for you. 
  • Support you to build the skills and confidence you need to be able to increase your independence. 
  • Negotiate with service providers about what they can offer you and how much it will cost from your plan funding. 
  • Support you in times of crisis and check in with you regularly. 
  • Provide the NDIS reports and assist you to prepare for review meetings. 

Our Support Coordination service operates independently of other Challenge Disability Services and manages potential conflicts of interest in an open and transparent manner.
